What we collect, which is very little.

This policy describes how East Florida Medical Clinic, PLLC, which operates ConfluxMed, handles information collected through this website at confluxmed.com. The site is informational. There are no accounts, no logins, no patient portal, and nothing here to sign up for — the only information we receive is what you choose to send us.

This is not our HIPAA notice. Once you become a patient, your health information is handled under our Notice of Privacy Practices, which we provide at enrollment and which is governed by HIPAA. Information you send through this website before you are a patient is generally not protected health information under HIPAA, which is precisely why we ask you not to send clinical details through it.

We collect two kinds of information, both minimal.

What you send us. If you use the inquiry form or email us, we receive your name, email address, phone number if you provide one, the state you live in, the topic you selected, and whatever you write in the message. The form composes an ordinary email from your own mail client — it does not store anything on this website.

Standard server logs. Like nearly every website, the hosting infrastructure records routine technical data when a page is requested: IP address, browser and device type, the page requested, and a timestamp. This is used for security and reliability, not to build a profile of you.

We do not run advertising trackers, marketing pixels, session recording, or third-party behavioral analytics on this site.

This site does not set advertising or tracking cookies. It may use strictly necessary cookies required for the page to function.

One disclosure worth making plainly: the site loads its typefaces from Google Fonts. When your browser requests those files, Google receives your IP address and basic request information, subject to Google's own privacy policy. We receive nothing from that request. If you'd prefer to avoid it, browser extensions that block third-party requests will prevent it, and the site remains fully readable.

We use the information you send only to:

• Respond to your inquiry.
• Determine whether you are eligible to become a patient.
• Schedule an intake visit if you decide to proceed.
• Keep the website secure and working.

We do not sell your information. We do not rent, trade, or share it with data brokers, advertisers, or marketing partners. We do not send marketing email to people who merely inquired.

We share the limited information described above only with service providers who help us operate — our website host and our email provider — and only to the extent needed to deliver those services.

We may also disclose information where required by law, to comply with valid legal process, or to protect the safety of a person. If you become a patient, sharing of your health information is instead governed by HIPAA and our Notice of Privacy Practices, which is considerably more protective than this policy.

Ordinary email and web forms are not secure channels. Please don't include diagnoses, medications, test results, or other clinical details in an inquiry. A sentence about what you'd like help with is plenty — the detailed conversation belongs in a visit, on the secure platform we use for patient communication.

If you send clinical information anyway, we'll handle it carefully, but we can't undo the fact that it traveled over email.

Inquiries that don't lead to enrollment are kept only as long as needed to answer them and for a reasonable period afterward in case you follow up, then deleted. If you become a patient, your medical record is retained under the retention requirements that apply to physicians in your state, which are longer and are described in our Notice of Privacy Practices.

You can ask us to delete the inquiry you sent, correct it, or tell you what we hold. Email hello@eflmc.com and we'll take care of it.

Depending on where you live, you may have additional rights under state privacy law, including rights to access, correct, delete, or limit the sale or sharing of personal information. We don't sell or share personal information for cross-context behavioral advertising, but we'll honor any applicable request all the same, and we won't treat you differently for making one.

This practice serves adults only, and this site is not directed to children. We don't knowingly collect information from anyone under 18. If you believe a child has sent us information, email us and we'll delete it.

If this policy changes, we'll update it here and revise the date below. Material changes will be described plainly rather than buried.
